Please use this form to record your attendance to an event. This form can be used by both individuals attending events on an individual basis, or by a nominated PLI Policy Member when multiple Members are attending. For Charity Dalek Squad facilitated events, this is to be completed by the Event Manager. For external Groups, such as Project Dalek, the lead facilitator for that event should complete this form.
